Ill Omen - I like this, 1/2 Approved!

Mortality Transcended - Thiiiiiis is a bit harder to decipher. While I think this is cool, the only thing that can be a bit tricky is what her undead state would mean for pvp rules and the Lethal Damage threshhold since losing an arm or loss of blood no longer incapacitates her. I'd like to see some sort of weakness here, even if its just as simple as 'destroying her brain is the only way to kill her' like in regular zombie movies. This is still cool though!

Shadow of an Ill Omen is approved.

Mortality Transcended exceeds the bounds of what is allowed for Ninja Abilities. As shown here, the effect cannot be defensive; immunity to illness and pathogens requiring living tissue can be construed as a defensive ability if it interacts with jutsu (other Medical jutsu coming to mind). All other aspects of the ability do not affect combat and are good by me.

Okay so...
- I supposed losing blood would no longer bring her to Death Threshold, but that's hardly worth a drawback as an A-rank? There are plenty of other ways to take someone down rather than through blood loss. Losing an arm, however, remains a handicap. While she might not bleed to death, the arm doesn't just regrow. Performing jutsu, fighting, etc... would still be nigh-impossible. Likewise, destroying the brain is a default sure-kill for just about anything. Is it really needed to state that?
- So for illness/pathogens, I'm mostly hinting at natural ones since indeed, this can not be affected by jutsu. It falls in line with Maik's ability that lowers physical damage by 1 rank (which can be caused by jutsu as well for that matter). I assumed the 'no jutsu' would have been a default clause no matter what you wrote down.
